Intermediate Tier Services are for Individuals in the community who may need support to gain independence or make decisions about their care following a period of illness or a stay in hospital.
The services include The Admission Avoidance Team, Bedded Units, The Care at Home Team, and The Reablement Home Support Team. It is planned with the carer and the patient at the right time, with the right people, in the right place. Many of you within the community and voluntary sector will be working alongside Intermediate Tier Services or support people following their involvement with these services.
